2020 TSMC Family Day: Guidelines for the Contact-Based Policy
To maintain steady domestic epidemic control, TSMC Family Day shall adopt the contact-based policy. When collection is required by epidemic control, the collected personal data shall be provided for use in epidemic investigation and contact by the health authorities with respect to the “Communicable Disease Control Act” and related laws and regulations. Personal data will be erased after 28 days since the date of collection.
Those who have read this operating principle and participated in the activity shall be deemed to agree to the above collection, processing and use of personal data.

In light of the fact that COVID-19 cases continue to grow and the Taiwan Centers for Disease Control (Taiwan CDC) has confirmed the domestic human-to-human transmission case. In accordance with Taiwan CDC’s suggestion and in order to protect the health of all the employees, visitors, relatives and friends of the Taiwan Semiconductor Manufacturing Company (“TSMC”), all people who are entering TSMC Family Day locations must cooperate to fill in the “TSMC Family Day COVID-19 Health Declaration Form”. Your personal data collected by TSMC via this “Family Day COVID-19 Health Declaration Form” contains your personal identification information (i.e., your name), physical condition, contact information, data concerning your social activities, and other data that is necessary for us to take anti-epidemic measures. Your personal data will not be used for any purpose other than the aforementioned epidemic prevention purposes (“Collection Purpose”). Pursuant to Article 3 of the Taiwan Personal Data Protection Act, you may have the following rights regarding your relevant personal data: the right to request access to the collected personal data, request a photocopy of the collected personal data, request supplement or rectification of inaccuracies in the collected personal data, request us to cease the personal data collection, processing or use, or delete the collected personal data. Any employee, visitors, relatives and friends who fails to complete the COVID-19 Health Declaration will be prohibited from entering all TSMC Family Day locations within Taiwan. The same consequence will apply if you request that TSMC cease the personal date collection, processing or use, or delete the collected data while the Collection Purpose is still applicable. Your filling in and submission of the Family Day COVID-19 Health Declaration indicates that you have consented to the contents set forth in this announcement regarding the personal date collection, processing, and use. This personal data collection is not mandatory for TSMC entities located outside of Taiwan.
